A QR code is just a picture — the squares are your link or text. There is no server in between, so nothing to host, nothing to expire, nothing to charge for. Sites whose "free" codes stop working after a week route them through their own redirect; that is how they switch them off. Codes made here point straight at your content.
Nothing. The code is built on your device, in this page. What you type is never sent anywhere or stored — load the page, switch off your connection, and it still works. Only your style choices are remembered, in your own browser.
Phones scan from about ten times the code's width: a 2 cm code from 20 cm, a 30 cm code from across a room. Keep the clear border — it is part of the code. Use SVG for anything printed and PNG for screens and documents. Raise the damage tolerance for small, laminated or outdoor prints.
